- Remove attrlist.[ch]::ntfs_attrlist_set because it was very bad idea to separate it from ntfs_attrlist_entry_{add,rm}.
...
- Update ntfs_attrlist_entry_{add,rm} and ntfs_inode_add_attrlist to work without it.
- Some other fixes, improvements and cleanups.
